Medical History Form

Below is the UIL Medical History/Pre-participation Physical form required for all UGHS Band members. For the 2026-2027 school year. Everyone will need to have the first page completed and turned in to Mr. Stafford. If you answer "yes" to questions 1-6 on the Medical History page, then you will need to be cleared by a doctor and complete the Pre-Participation Physical (Page 2) prior to starting any marching activity. Also, if you are new to the UGHS Band or did not have a physical (for athletics or otherwise) during the 2025-2026 school year, please be sure to have this completed prior to the start of marching rehearsals on Tuesday, July 21, 2026.
